posix-entry-ops: do not copy when reading xattr
The code is simplified to avoid needless copy as well as simplified
overall for readability.
Such changes are needed elsewhere too (see
https://github.com/gluster/glusterfs/issues/720 )
Few other minor changes here and there, nothing functional.
